Medical records consists of information kept by ER24 about the medical condition of the patient. 

Why do we keep medical records?

• Patient information and billing
• Documentary evidence of the event and the patient's treatment received
• General information for case study and evaluation of the treatment rendered to the patient
• The protection of medico-legal interests of the company and the patient

Can I access my medical records at ER24?

• Yes, a patient or a legal representative  can access his / her medical records at ER24*

How do I access my medical records at ER24?

• The patient or a legal representative needs to file a written request with ER24. This can be done by sending an email to CQC
• If the patient request his / her records, a certified copy of the patient's ID need to accompany the request.
• A legal representative can also apply for the records once a valid Power of Attorney Letter is attached.
• Please note an administration fee applies to all successful applications.

* Please note that Medical Records are regularly archived and records depending on the service date might take a few days to be released.
