ER24 runs the BokSmart Spineline, a service dedicated to improving recovery outcomes for rugby players who suffer head and spinal injuries on the field. 

BokSmart is a national initiative designed to improve rugby safety at all levels, from school games to professional matches, ensuring a safer experience for players.

“The programme focuses on educating coaches and referees about injury prevention and management,” says Heinrich Africa, ER24’s Clinical Performance Operations Specialist. “Particular attention is paid to serious conditions like concussions, head injuries, and spinal injuries that can have life-changing results if not treated appropriately.”

How ER24 operates the BokSmart Spineline

A key component of this nationwide initiative is the BokSmart Spineline, a dedicated emergency helpline managed by ER24. “The aim of this 24-hour service is to provide appropriate pre-hospital emergency medical care for players who suffer serious head and spinal injuries,” says Africa. “One of ER24’s core values are that we’re ‘team orientated’ – and the game of rugby provides an analogy for how working together can help everyone achieve the best end goal.”

ER24 paramedics present at rugby matches, from school games to Super Rugby, Sevens, and international matches, are trained in World Rugby’s BokSmart evidence-based safety guidelines. This training helps them quickly recognise serious injuries and take the right action.

Prioritising patient safety

“Our primary focus is concussions, head injuries, and spinal injuries due to their severity and potential difficulty in immediate identification,” says Africa. “ER24 paramedics will look for signs such as confusion, unsteadiness, irritability, dazed appearance, or convulsions.”

Also, training drills for spinal immobilisation and other situations are conducted before each match. “This is to ensure each team member knows their roles and responsibilities on game day and that we’re ready when needed,” Africa says. “This is vital as their effective response can positively impact the player's future wellbeing and potentially minimise long-term complications.”

To qualify for this service, the injury must be reported directly to the BokSmart Spineline. When contacted, ER24’s emergency operators at the Contact Centre will assess the injury and quickly send an ambulance with the right crew and equipment.
